Form Name: Application for Student Organization Recognition
Use For: Information on how to start a new student organization at UMKC. Please also read - How to create
a constitution for student organizations.
Form Name: Area/Equipment Request Form
Use For: This form should be completed for any event or activity that will take place outdoors.
Form Name: Student Organization Interest Form
Use For: This form should be completed if you would like us to send you additional information on one of the listed student organizations on campus.
Form Name: Demonstration/Rally Permit Request Form
Use For: This form should be completed when your organization would like to hold a demonstration or rally.
Form Name: Dance/Social Event Form
Use For: This form should be completed for any dance or social event that is scheduled on campus. Please also review the Dance/Social Event Policy.
Form Name: Display Case Reservation Form
Use For: This form should be completed to reserve the display case located outside of the UMKC Bookstore.
Form Name: Facilities Request Form
Use For: This form should be completed to reserve any room or space in a University building excluding the University Center. To reserve space in the University Center call (816) 235-1416.
Form Name: Grade Release Form
Use For: This form should be completed whenever your organization needs to verify student grades or GPA.
Form Name: International Flags Check-Out Form
Use For: The Student Life Office has international flags that can be reserved for student organization use.
Form Name: Money Collection Form
Use For: You are required to complete this form and abide to the Money Collection Policy if your organizations has received funds from SGA Appropriations and will need to collect money at your events. .
Form Name: Officer Listing Update Form
Use For: Student Organization leaders should complete this form and submit it during the first five weeks of each semester, or whenever there is a change in officers or contact information.
Form Name: Permit to Sell
Use For: This form should be completed whenever there will be cash exchanged for a product or service.
Form Name: Poster Request Form
Use For: Enlarge your 8x11 flyers to banner size. Cost: $10.00 each. Use this form to submit your request.
Form Name: Sound System Check Out Form
Use For: Complete this form to reserve a sound system for use at your student organization event in the University Center or University Playhouse/University Center Walkway. The large sound system comes with 2 large speakers, 2 CD players, 2 tape decks, 1 wireless microphone and requires 2 operators from your organization. The small system comes with 1 CD player and 2 wireless microphones. Please also see the Sound System Usage Policy.
Form Name: Student Organization Listserv Request Form
Use For: Complete this form to set up a listserv for your student organization, or to make changes for an existing listserv. Please print and submit form for signature approval to Student Life Office.
Form Name: Student Organization Webpage Request Form
Use For: Complete this form to set up a website or email address for your student organization, or to make a changes in who may access the website or email address. Please print and submit form for signature approval to Student Life Office
Form Name: Tax Exemption Form - Missouri
