Whatever you want to do, there's probably some paperwork to do first. Some of the forms most pertinent to Greeks are available below. There are others for all student organizations on the main Student Life site.
Open the link you need. You may type directly through your browser, but you will need to print the completed form and bring (G-6 University Center) or fax (816-235-5590) it to the Student Life Office.
Officer Listing Update Form
This form is not necessary for Greeks. Update this in G-MIMS .
Area/Equipment Request Form
This form is required for anything that happens outside at the University (i.e. not indoors, but on campus). If you want to use University Playhouse for an event, complete the form. If you want to hang up banners outside, complete the form. If you want to chalk sidewalks, complete the form. Do not even think about chalking anything without completing the form under penalty of severe pestering by your Greek advisor.

Facilities Request Form
This is for anything inside on campus (i.e. within a building). This works for every building except the University Center. Call 816-235-1416 if you want to reserve space in the University Center. Do it as far in advance as possible.
Dance/Social Event Form
If you're holding a dance on campus , you must complete this form. Also, you should read the Dance/Social Event Policy . There are a number of rules that pertain only to dances and must be followed strictly by all campus organizations.
Display Case Reservation Form
You can reserve the display case in the cafeteria next to the bookstore. This is good for getting out recruitment or public relations information .
Alcohol Social Event Registration Form
If you are having alcohol at an event , whether on campus or off, you must complete this form. Also, read UMKC's Alcohol Social Event Policy . Remember you risk management education and keep in mind that Missouri law prohibits anyone under the age of 21 from possessing or consuming alcohol. This has nothing to do with UMKC; talk to your congressperson.
Permit to Sell
If you're collecting any money on campus, whether for profit or for charity, you must turn in this form first.
Sound System Check Out Form
We have a stereo system you may use for events in or near the University Center. You will need to be trained to use the system, so work ahead. Guess what, there's a Sound System Usage Policy .
Student Organization Webpage Request Form
You all have websites already, but follow this link to learn how to develop it.
Grade Release Form
This form is no longer necessary.
Greek Excellence Award application
The Greek Excellence Award was established by Greek Advisory Board to recognize and support chapter development. Chapters complete the application form (including necessary documentation) and are assigned points, which translate to one of three award classificiations. Awards are presented at the April Greek Week Banquet.
